Mental Health and Substance Abuse Treatment Team:
Keith Gardner, LMSW, IAADC
Keith has over 35 years of work experience in the substance abuse and mental health field. Keith holds a Bachelor’s degree in Social Work from the University of Wyoming and a Master’s degree in Psychology from California Coast University. He is a Licensed Master Social Worker (LMSW) and is certified International Advanced Alcohol and Drug Counselor (IAADC). He especially enjoys working with relationship issues in recovery and groups. Keith works at Crossroads Behavioral Health Services out of our Creston and Winterset offices. Keith has held multiple positions in his career as therapist, professional development trainer account manager/marketer, and field director for Idaho and Montana prison ministries. Keith has been with Crossroads since 2013.
Medication Management Services:
Vickie Paulus, RN
Ms. Paulus started at Crossroads in June of 2009. She has been an RN for over 30 years, working in the medical field for over 40 years. Vickie works with multiple doctors at Crossroads and helps with the Telehealth to be able to see clients via the computer and assists clients. She is also available for questions regarding medications, patient assistance program and medication follow-ups.
Kovilparambil Antony, MD
Dr. Antony started with Crossroads in May 2014. He earned his bachelor of Medicine in 1995 from St John’s Medical College. He has been a board eligible psychiatrist with over 20 years of experience in both inpatient and outpatient care. Skilled in psycho-pharmacological management and psychiatric disorders. Currently Dr. Antony serves clients at the Creston location.
Timothy Jack, MD
Dr. Jack started working with Crossroads in 2015. He completed medical school at the University of Maryland in 1984. He did his psychiatric residency at the Sheppard and Enoch Pratt Hospital in Baltimore and earned his board certification in psychiatry in 1990. He currently provides psychiatric services in the Creston location.
Kim Spears, ARNP
Ms. Spears has provided services with Crossroads since 2015. She earned her Bachelor of Science in Nursing at Boise State University and her Masters in Nursing at Stonybrook University, Long Island. She spent six years working at an inpatient psychiatric hospital and has been practicing as a Psychiatric Mental Health Nurse Practitioner since 2007. Kim currently assists patients in our Creston and Winterset offices.
